# How to check the “Original form” of the input document

To check the events that occurred in the input document, the original form is required.\
After checking the revision of the original form in ConMas Manager, Please output it from\
ConMas Designer and send it to us.

### How to check the “Original form” of the input document

1\. Select the "Documents" tab in ConMas Manager and select the input document from the list.\
　Check the "Form ID" and "Form Name" listed in "▼Basic Information".

2\. Select the "Forms" tab, search for the "Form Name" you checked in step 1 from the list of form,\
     and open it.

3\. Confirm that the "Form ID" listed in "▼Basic information" is the same as the "Form ID"\
     checked in step 1.

#### 　→If the Form ID is the same

　　4‐1．Please output the form from ConMas Designer.

#### 　→If the Form ID is different

　　The input document in which the problem occurs may have been created from a previous revision \
        of the form.

　　4-2. Please check if there is a matching "Form ID" in "▼History" at the bottom of the\
                 "Basic Information" screen of the form.\
　　　　If there is a matching "Form ID", please output the form of the revision (**past revision**)\
                using the steps described on the next page.

If you are using , when you select an input form in the form list of the i-Reporter app,\
You can check the "Form ID" from "Information" at the bottom of the right screen.

Please refer to this when opening the input document in ConMas Manager.
